NEW Face To Face Attendance Verification Form Request Protocol

Dear Parents/Guardians: 

All attendance verification letter requests are to be made with our school staff member, Robin Anderson at randerson17@schools.nyc.gov.  This includes letters for: 

• IRS
• HRA
• Housing
• Bank, etc.

ONLY PARENTS/GUARDIANS CAN PICK UP THIS FORM.  ALL PARENTS/GUARDIANS MUST PROVIDE A VALID ID THAT MATCHES OUR RECORDS SYSTEM.  THIS IS REQUIRED BY THE NYCDOE.

VALID IDs ARE ONE OF THE FOLLOWING:

• NYS LICENSE
• NYS ID
• PASSPORT
• GREEN CARD

Please note: anyone that enters the building MUST FOLLOW ALL SCHOOL SAFETY PROTOCOLS.
